Community Home Care’s home health care referral services in Brooklyn, NY, provide the level of care that home-bound patients deserve, matching their unique needs to highly trained home health aides or helping CDPAP applicants deliver personal care to their family members and friends.
All enrolled Veterans are eligible for Homemaker Home Health Aide Care IF they are eligible for community care and meet the clinical criteria for the service and it is available. Services may vary by location. A copay for Homemaker and Home Health Aide services may be charged based on your VA service-connected disability status.
These services are referred to as home health care and are considered extended care services prescribed by and under the direction of a VA physician. VA provides several types of home health care including: Veterans can receive more than one type of service at a time.
